The Glasgow Coma Scale for adults, and its derivative the Glasgow Coma Score (GCS), is scored between 3/15 (the worst) and 15/15 (the best). The GCS is composed of three parameters: best eye response, best verbal response and best motor response.

The GCS should be recorded and communicated as follows: The denominator should be specified when recording or communicating the total score. For example: a GCS of 13 would be recorded as 13/15

In addition to the total score, the individual components of the GCS should be described. For example a GCS of 13/15 based on scores of 4 on eye-opening, 4 on verbal response and 5 on motor response should be communicated as E4, V4, M5.
